Item Appliance of CAD / CAM modeling in prosthetics of bone defects of facial bones(Aluna Publishing, 2021-06) Grigorov, Serhii; Григоров, Сергій Миколайович; Khudyk, Anton; Худик, Антон Каренович; Sukhina, Iryna; Сухіна, Ірина СергіївнаThe aim: Improving the ability to restore the shape and function of the bones of the maxillofacial area through the use of their own techniques. Materials and methods: Materials and methods: This research was carry out on the basis of the Department of Surgical Dentistry and Maxillofacial Surgery of Kharkov National Medical University in the Department of Head and Neck Surgery, Kharkov “Regional Clinical Hospital” (2018-2020 years). Examination and treatment of 26 patients, aged from 19 to 55 years, who needed reconstructive surgery to restore the anatomical shape, integrity and function of the maxilla or mandibular jaw. Results: All patients were treated according to the developed method (utility model patent № 145754 “Method of treating traumatic fractures of the maxillofacial area in adults using bone osteosynthesis 3D modeled titanium mini plates”), which consists in 3D modeling, based on previously performed computed tomography, and exploitation individually modeled titanium mini grids and titanium endoprostheses. The experience of this technique shown the advantage of the proposed method not only in the correction of traumatic defects, but also defects of the jaw bones that occur due to the removal of bulky neoplasms. Conclusions: Individual 3D simulated mini grids / plates and endoprostheses, according to the study provide maximum adaptation and restoration of anatomical shape, relief of jaws, their integrity and contour, provide prevention of pathological fractures by stabilizing residual bone tissue (titanium mini grids) serves as a carcass , provide prevention of prolapse (germination) of soft tissues in the area of postoperative bone defect (mini grid does not allow soft tissues to germinate in the area of postoperative bone defect on the outside), which improves functional and cosmetic results. Thus, our proposed method of osteosynthesis using 3D simulated titanium miniplates can be recommended for use in clinical practice.Item Bases test for KROK-2. Materials and methods: In order to select and assess the most informative diagnostic criteria for predicting the treatment results for various OMS forms, the total of 9 subject matter experts (SME) were included into the problem commission on the specialty “Dentistry”. Results: After calculating the data obtained according to the method of Yana V. Nosova, the working group experts’ level of competency was M = 0. 90. This confirmed the group’s qualification, which further led to the approval of scoring coefficients, depending on the degree of a particular index importance. The basic and minor parameters in the subjective, objective, introscopic and laboratory data of OMS patients were identified. The study included 81 patients with injuries of facial crani- um, which were divided into 2 groups, depending on the pres- ence of complications. The following enzyme indicators were © GMN studied: the level of excretion of hydroxyproline in daily urine; alkaline and acid phosphatase activity; the percentage of bone isoenzymes of alkaline phosphatase. To assess the mineral me- tabolism, the level of total and ionized calcium and inorganic phosphorus in the blood serum, as well as their excretion in the urine, were determined. To assess the state of metabolism, the concentration of glycosaminoglycans and their fractions in the blood serum were studied. To study the structural and functional state of the bone tissue the densitometry was performed. In patients with complicated course of injuries of facial crani- um assosiated with traumatic brain injury there was revealed the increase (р<0,05) of: excretion of phosphorus, uronic acids and oxyproline, while the excretion of calcium was not disturbed (р>0,05), and excretion of magnesium was decreased (р<0,05). It was found out that the level of calcium of blood serum in patients with complicated course is significantly (р<0,05) lower than in the control group and does not depend on the presence of craniocerebral injury (р>0,05). The decrease of the level of ionized calcium content in blood serum can be the confirmation of lower metabolic activity of reparative osteogenesis process- es, first of all at the expense of damage of central mechanisms. When studying the content of carbohydrate–protein metabolites by complicated course of injuries of facial cranium, the absolute increase (р<0,05) of concentration of chondroitin–6–sulfates was revealed, and during the analysis of results it was found out that in absolute values, as well as in structural indexes, the specific weight of various fractions changes, that can be the evi- dence of instability of mechanisms of osteogenesis and of dam- age of physiological mechanisms of reparative osteogenesis. The results of 153 patients’ (prospective group) treatment were analyzed with the use of medical expert systems (MES), gender - age distribution, clinical - anamnestic data, type of surgery and comparative characteristics of complications with retrospective group. Reliable computer diagnoses were obtained, suggested by the MES, which coincided with the final clinical diagnoses in 97% of cases and individualized comprehensive treatment was applied to each clinical case. By means of MES, with regard to the prognosis of diagnosis and the comprehensive treatment variant, we have managed to reduce the proportion of actual complications depending on the clinical OMS form from 9, 22% (19 patients) to 2.6% (4 patients). The initial results analysis established the efficiency and prospects of using the automated “Easy-sinus 1.01” MES in the treatment of patients with various OMS forms.Запропоновано впровадження в клінічну практику автоматизованої комп'ютерної програми прогнозування перебігу захворювання та лікування пацієнтів з різноманітними формами одонтогенного верхньощелепного синуситу (ОВС). Проведено аналіз результатів лікування 153 пацієнтів (проспективної групи) з використанням медичної експертної системи (МЕС), розподілу за гендерно-віковими, клініко-анамнестичними даними, типу хірургічних втручань і порівняльної характеристики ускладнень з ретроспективною групою. Отримано правдоподібні комп'ютерні діагнози, які були запропоновані МЕМ, збіглися з заключними клінічними діагнозами в 97%, що в подальшому дозволило застосувати індивідуалізоване комплексне лікування в кожному з клінічних випадків. За допомогою застосування МЕС для прогнозування діагнозу і варіантів комплексного лікування вдалося знизити питому вагу фактичних ускладнень в залежності від клінічної форми ОВС з 9,22% (19 пацієнтів) до 2,6% (4 пацієнта).
